Intake Forms

Forms must be completed 24 hours before your first appointment. This provides your doctor adequate time to review your file and prepare for your first visit.

Direct Billing

We offer direct billing for Motor Vehicle Collisions (MVA/MVC), RCMP, and Seniors Blue Cross only. For all other cases, we provide receipts for you to submit to your private insurance provider.

Payment for Services

All of our services require payment up front and we provide you with a receipt for your insurance claims. We accept all forms of payment.

Location

We are located directly behind Superstore off the QE2 (Highway 2). Take the third exit going North out of Calgary or the first exit going South from Red Deer (Veterans’ Boulevard). The clinic is located near Wanderwash and Dr. Bell’s Dental Clinic, and across from the three-story Aqua apartment complex.
